Your records and complaints

Last updated October 15, 2026

Texas law asks every health care practice to tell you how to get a copy of your records, how to contact the authority that licenses its practitioners, and how to make a complaint. Here is how, for Lavender Meridian.

Request your records

Ask us in writing by email to lyndsie@lavendermeridian.com or by mail to Lavender Meridian, 314 1/2 Congress Ave, Austin, TX 78701. You can also call 808-936-7806 to ask how. Tell us your name, the email address or phone number you used with us, and which records you would like. We confirm who you are before we send anything, and we respond to written requests within 15 business days.

Contact the licensing authority

Acupuncturists in Texas are licensed by the Texas State Board of Acupuncture Examiners. Complaints about a licensed acupuncturist are investigated by the Texas Medical Board.

  • Online: www.tmb.texas.gov, under File a Complaint
  • Complaint line: 1-800-201-9353
  • General questions: 512-305-7030
  • Mail: Texas Medical Board, Attention: Investigations, P.O. Box 2018, Austin, Texas 78768-2018

Please include the practitioner's name (Lyndsie Lavender, L.Ac.) and the practice address: Lavender Meridian, 314 1/2 Congress Ave, Austin, TX 78701.

Make a complaint about your health information

If you are concerned about how your health information was handled, you can tell us first and we will respond. You can also contact the Office of the Texas Attorney General, which lists the state agencies that handle health privacy complaints and how to reach each one, at www.texasattorneygeneral.gov (Consumer Protection, Health Care).
